A command is not proof of an open path

Problem

A start command can be issued without independent confirmation that the required process path is open. The command state therefore cannot stand in for position feedback.

Solution

Review the permissive against independent open-position proof and identify the missing condition in the logic.

Before any change, define the approved response to missing or lost feedback, timeout and alarm behavior, and the field checks needed to validate it.

Result and evidence limits

A design gap and a proposed direction are identified. A field test or implemented correction is not recorded; reliable operation after a change is not asserted.
